MEMBERSHIP CHANGES

To request changes to your membership, such as freezing, unfreezing, terminating, or updating your contact and payment information, please complete the form below. Be sure to list the member(s) or participant(s) for whom the changes apply.
Important Notes:
 
  • All membership changes must be submitted online at least 72 hours before the end of the month to take effect for the next billing cycle.
     
  • Freezing the primary member on a couples or family plan will NOT freeze all the members on the contract.
     
  • If the primary member on a couples or family plan terminates, all members on the plan will also be terminated.
